Replacing/Cancelling Lost Identification and Cards BIRTH CERTIFICATES: Send the following information to: Vital Statistics 254 Portage Avenue Winnipeg, MB R3C 0B6 Information on their Website: http://vitalstats.gov.mb.ca Full Name, Place of Birth, Father’s full name, Mother’s maiden name, Size of card required (wallet size or 8” x 10”) with a $25.00 Cheque/Money Order payable to the Minister of Finance NOTE: There is NO fee if your Birth Certificate was stolen - see the attached information (You must provide a Police Incident Number to have this fee waived)
When replacing/reporting lost cards to a bank, you must provide them a sample of your signature.
